Patient Referrals

Your time as a provider is valuable. Providers often encounter patients that are experiencing depression, anxiety, grief, or have gone through some trauma. Patients may show up to their appointment overwhelmed and ready to unload their burdens. Given that you have a full schedule of patients to attend to, you may not have the adequate amount of time to provide the support needed. By submitting a referral through our HIPAA compliant referral form, you can provide your patient with reassurance that they will have a follow-up regarding their mental health related concerns.

The Process

1. Fill out our HIPAA compliant referral form.

2. Your patient will receive an email or text with instructions to schedule their free 15-min mental health consultation.

3. During the consultation call, insurance will be verified and if it is determined that it is a good fit, an intake evaluation will be scheduled.

4. Provider will receive an updated email once patient has been scheduled.
